Preparing
for your exam (click here)
Many exams require special preparations. Please check with
your referring physician if any special preparations are necessary.
You can also find exam prep instructions on the reverse side
of CMI’s prescription order given to you by your doctor
or by clicking on a specific exam under Services. If you have
additional questions regarding your exam or preparations, feel
free to ask our scheduling or registrar representative.
For your convenience, if you would like to pre-register,
call our registrars office at 352.253.0150 or 352.253.0771.
If you would like to pre-register at the Center, please arrive
20 minute prior to your scheduled appointment. As a courtesy,
if you must cancel or reschedule, please call within 24 hours
of your appointment. Upon arrival, you will be asked for
a valid Driver’s License (or other form of a photo
ID), your current insurance card, and your physician’s
order form (provided by your referring physician – if
you do not have a copy of the order, please make sure a copy
has been faxed to the Center for Medical Imaging prior to
your arrival). In some cases you may need to bring past films
for comparison. As an affiliate of Florida Hospital Waterman,
CMI has access to all previous scans done at the hospital.
Billing & Insurance

As a convenience to you, we will file your claims directly
with your insurance company and will assist you in obtaining
the maximum payment for a given procedure. The Center for
Medical Imaging is a Medicare approved facility and accepts
assignment on all Medicare claims. Any balance not paid
by Medicare is forwarded to your secondary insurance company,
where most times, the balance is generally paid in full.
We also participate in many managed care plans accepting
payment in full for covered services of those plans which
do not have co-insurance or deductible requirements. Click

You are responsible for any known co-payment or deductible
not satisfied at the time of your exam, as well as any non-covered
services. When time allows, you will be notified of any payment
you need to bring with you to your appointment. Once payment
is received from the insurance company, any unpaid balance
is forwarded to you. In the event that your insurance company
does not make payment within 45 days, we will send you a
balance statement. Ultimately, it is the patient’s
responsibility for any unpaid balance. Your understanding
and support is greatly appreciated. Should you have any questions
